Young Carer Planning and Assessment Officer

This organisation is a long standing not for profit organisation based in a central Perth location. Working as part of a national association they provide a range of support services to the community. This role sits within a federal government funded program and works specifically with youth. This relatively new service has an established team of mental health clinicians who each handle a caseload of clients.

Your new role
As the Assessment and Planning Officer you are responsible for handling a community based caseload, providing assessment and support planning services. Reporting to the Program Manager you will have support for any complex cases and reviews. In this role you will enjoy working in a collaborative environment with our social care professionals. Key responsibilities include:

  • Provide supports for young individuals and their families
  • Work within a primary mental health care model
  • Conduct intake, assessment and planning sessions
  • Maintain case notes and data entry
  • Actively participate in clinical supervision and team meetings
  • Advocate and raise awareness for clients needs in the wider community
  • Develop key stakeholder relationships
  • Develop relevant tools for client stress management and self care

What you'll need to succeed
In order to succeed in this role you will be a tertiary qualified healthcare professional with proven experience in a similar role. Key requirements include:

  • Tertiary qualification in mental health, youth work or social work
  • Proven experience working with vulnerable people
  • Experience providing assessment and planning services
  • Ability to perform advocating duties
  • Well developed understanding of trauma informed practice
  • Demonstrated ability to work in an outreach environment
  • Desirable: ASIST training
  • WWCC
  • National police clearance
  • Current C Class Drivers Licence
